Box 33a and 32a will be the NPI Group NPI if you have one, if not use your individual NPI number. 24 J-box bottom or un-shaded will contain your Individual NPI number. No other numbers; 32b, 33b or the J-Box top or shaded can have any numbers, you will not get paid if there is a number in these fields.
